Zapalenie zatok przynosowych w pierwotnych niedoborach odporności

Zapalenie zatok przynosowych należy do najczęstszych i istotnych klinicznie manifestacji pierwotnych niedoborów odporności (PNO), zwłaszcza niedoborów humoralnych, które stanowią około połowę wszystkich PNO. Pacjenci z niedoborami przeciwciał są szczególnie podatni na nawracające i oporne na leczenie zapalenia zatok, których przewlekły charakter często opóźnia rozpoznanie współistniejącego zaburzenia odporności. Celem niniejszej pracy jest przegląd aktualnej wiedzy na temat epidemiologii, patofizjologii, diagnostyki oraz leczenia zapalenia zatok przynosowych w przebiegu PNO.
Częstość niedoborów immunoglobulin jest istotnie wyższa u chorych z nawracającym (13%) i opornym na leczenie (23%) przewlekłym zapaleniem zatok przynosowych (PZZP) niż w populacji ogólnej, co uzasadnia diagnostykę immunologiczną w tych grupach, zgodnie z wytycznymi EPOS 2020 i ICAR-RS 2021. Patogeneza ma charakter wieloczynnikowy i obejmuje dysfunkcję bariery nabłonkowej, upośledzenie opsonizacji i fagocytozy bakterii otoczkowych, zaburzenia odporności wrodzonej oraz rolę biofilmów bakteryjnych. Omówiono najważniejsze niedobory humoralne związane z zapaleniem zatok: pospolity zmienny niedobór odporności (CVID), selektywny niedobór IgA, niedobór przeciwciał swoistych (SAD), niedobór podklas IgG, agammaglobulinemię sprzężoną z chromosomem X, zespół hiper-IgM oraz zespół aktywowanej kinazy PI3K delta. Podstawą leczenia swoistego pozostaje terapia substytucyjna immunoglobulinami u kwalifikujących się chorych, zmniejszająca częstość i ciężkość zakażeń. Leczenie uzupełniają celowana antybiotykoterapia, profilaktyka antybiotykowa, szczepienia oraz – w przypadkach opornych – endoskopowa chirurgia zatok przynosowych. Kluczowe znaczenie mają wczesne rozpoznanie niedoboru odporności, zapobieganie nieodwracalnym powikłaniom płucnym oraz wielospecjalistyczna współpraca laryngologów, immunologów i pulmonologów.
